The Milford Band programs follow school district policy for the administration of prescription and non-prescription medication to students. Depending on the type of medication, the following forms must be completed and placed on file in the school office.
- The Non-Prescription Medication Form can be completed and signed by a parent or guardian for over the counter medications (e.g. Tylenol, Motrin).
- The Prescription Medication Form must be completed and signed by a physician. The form and medication (in its original container) must be given to one of the directors by a parent or guardian. The medication is kept in a locked area and accessible by the directors and Milford Schools staff.
- The Self Medication Form is specifically for students who need to carry EpiPens and inhalers on their persons. The form must be completed and signed by a physician and turned in to the directors. Please remember that per district policy, no student may self medicate at school outside of EpiPens and inhalers.
In addition to the above forms, please inform the band directors and head chaperone if your child has a specific medical need (e.g. food allergy, asthma, etc.)
